The Hospital Council commissioned member-approved consultants to conduct reports on the Economic Impact of Northern & Central California hospitals. The purpose of these reports is to estimate the economic impact of hospitals and hospital-related spending in the region. These reports are critical to both our public and political advocacy.

The Endorsed Business Partner Program was established to provide cutting-edge and cost-effective solutions for our member hospitals. The vendors selected are expected to meet high quality standards and offer products or services that help health care facilities avoid costs, recover revenue, reduce operating and capital expenses, improve management and quality, increase productivity, develop staff resources and apply new strategies.

African American Health Disparities Project
The African American Health Disparity Project
The African American Health Disparity Project was formally launched in November 2002 by Co-Chairs: Martin Brotman, MD, President and CEO of California Pacific Medical Center; Mark Laret, CEO of UCSF Medical Center; and Anthony Wagner, Vice President, Office of Labor Management Partnership, Kaiser Foundation Health Plan, Inc. The infrastructure of the Project was designed as a collaboration in which all members have equal input into fact-finding, problem solving, program development and implementation.

The 2011 CMTI participants experienced ROIs ranging from $50,000 to more than $1 million annualized revenue increase and cost decreases. This exciting program assesses each facility’s current program and then works to define metrics, goals and measurable results. This program utilizes a standard data download from each facility to analyze current needs, opportunities and Best Practices and then defines goals for your hospital to ensure the program is meaningful among a diverse group. The Hospital Council brings hospitals together to collaborate on the community health needs assessments required under the State’s community benefit law. These reports show the health status of the communities our hospitals serve across a range of health indices and are a publicly available resource for all to use.

Medical Respite
Medical Repsite
Leaving the homeless patient in the hospital to recuperate is the most expensive location when they are ready for self-care. The Medical Respite program gives the homeless patient a safe, clean environment for recuperation and provides an ideal situation for linking the individual to entitled benefits. Case workers assist them in obtaining SSI and Medi-Cal, which will be the stepping stone to getting them into permanent housing and ongoing medical care.
New Directions
New Directions
An Intensive Case Management Program
New Directions is an innovative hospital and community-based case management program for individuals with complex medical and psychosocial needs. A program of the Healthcare Foundation of Northern and Central California, New Directions is dually focused on both business concerns and patient care. The business focus is to reduce the impact of frequent users of the emergency departments at Santa Clara County hospitals. The patient care focus is to improve the quality of health and lives of frequent users of ED services through improved access to medical care and social services.
Since its inception in 2002, New Directions’ intensive case management services have helped stabilize the lives of more than 250 of our county’s most vulnerable patients- those who have complicated medical conditions or are at risk from the medical complications resulting from homelessness, lack of primary care or a medical home, lack of financial or health insurance benefits, lack of transportation, addiction to drugs or alcohol and/or untreated mental illness.
New Directions’ philosophy is to do “whatever it takes” to stabilize and support these patients. Services are provided by culturally competent, Masters level social workers and include: individualized assessment and care planning; meeting immediate needs for food, shelter and clothing; assistance with accessing medical care, income or benefits, housing, mental health services, drug and alcohol support, assistance with legal issues, transportation, and ongoing supportive services.
Funding for New Directions originally came from the generous support of the Heath Trust, the California Healthcare Foundation and the California Endowment. In recent years local hospitals recognizing the positive return on investment, have funded the project on a case rate basis. Additional funding from community organizations such as Kaiser Foundation, and Pacificare has enabled New Directions to provide case management services to patients utilizing the Medical Respite program at EHCLifebuilders, while funding from Destination: Home has enabled services for referred clients from Santa Clara County’s One-Stop Homelessness Center. Strong collaborations with community and county agencies ensure comprehensive services and reduce the duplication of efforts involved in meeting the needs of these patients.
