LFS Application Template to Train CLSs

By offering clinical training sites, hospitals have the opportunity to partner with CLS education programs and to “grow their own staff.” In California, hospitals must have approval from the California Department of Public Health, Laboratory Field Services (LFS) to train.
In response to requests for assistance in filling-out the LFS application to train, the HLWI has prepared a summary of items needed to construct the application binder and a step-by-step application template. The template is specific to CLS programs in the California State University system.
