Medication Safety Symposiums are planned and scheduled in cooperation with the California Department of Public Health and CDPH’s team of Pharmaceutical Consultants.  The objective is to create peer-to-peer learning networks that support the sharing of ideas and strategies that enhance medication safety in healthcare delivery along the continuum.  Regional meetings provide an opportunity for hospital pharmacists to ask questions and receive information from CDPH staff, our state colleagues.  They also provide opportunities for hospitals to share their strategies, tools and ide

as to improve medication management.  In conjunction with CMS Region IX  and the Partnership for Patients, the following goal has been agreed to: Improve Medication Safety across transitions of care such that 100% of patients enter a new care setting with an accurate list by August 2012.
